PTSA Tax Exempt Form. As a 501(c)(3) non-profit entity the ARS PTSA is exempt from paying sales tax for PTSA related purchases. Just download and print out this form, sign and date it, and then present it to the merchant to request that sales tax not be charged.
PTSA Reimbursement Voucher Form. Use this form to get reimbursed, or get a check for ARS PTSA related expenses (no taxes will be paid). Fill out the form and turn it in with the attached receipts to the PTSA mailbox folder in the front office at the school. Our Treasurer will arrange to get you your check.

Building Use Application Form. Please fill out this form if you are planning an non school group event at the school. Turn it in at the front office of the school. Included in this form is a TFE (Temporary Food Establishment) permit application, which is required if you will be providing food to the public.
